Conventional cotton is one of the most pesticide-intensive crops grown, and underwear sits against mucous membranes and thin skin for most of the day. Organic cotton is the same fiber grown without those synthetic pesticides, which is a small change everywhere else and a meaningful one here.

Can you get a bra with no synthetics at all?

Yes, though the selection is small. Elastic-free and latex-free bras exist, built from 100% organic cotton with no elastane in the band, and they are the option people with latex sensitivity or contact irritation usually end up at. They fit differently from a stretch bra by design.
